Need A Gown or know someone who does?
To qualify for the boutique, participants must have a referral
form completed by a guidance counselor, teacher, church leader
or social service representative. The application period is
from February 1 to April 1.
All forms will be reviewed and referred candidates will receive
notification in early April. Our goal is to provide an opportunity
to all qualified participants. Participants are expected to
call and confirm their attendance at the boutique.
Referral forms are available on the forms
page of this website.
